If you need fresh labs, your provider provides a requisition form for Quest or LabCorp so you can walk in without paying the lab directly. You pay a flat $150 that covers the requisition. Follow-up labs at 6 weeks, 3 months, 6 months, and annually are included in your monthly subscription at no extra cost. FedEx Overnight shipping ($15 per shipment) is billed separately. No insurance is filed; we can provide a superbill on request for possible out-of-network reimbursement.
